Printable surveys

Is it possible to have a pop-up calendar?

Is it possible to have a pop-up calendar as a question response option? How about a drop-down list? I'd like to send the same survey to the same people every day, and get the results weekly. What is the best way to organize this?

wrong display of the new page

When you create a new empty page, the "(* indicates that a response is required)" is still there even if there is no question there....

Multiple choice: Limiting selections

For multiple choice items, it appears that settings can be changed between "Select one" or "Select all that apply." Would there be a way to limit the number of selections (e.g. "Select top 3 choices")? Thank you.
